Pre-surgical Consultation

Your pre-surgical consultation includes a comprehensive evaluation of your dental needs, as well as a thorough review of any medical conditions and current medications. We also encourage you to discuss any concerns you may have prior to your surgical procedure during this time.

The following steps will help you come prepared for your first appointment with us:

• Please bring all insurance (dental and medical) information with you.
• At your request, your referring dentist may send current x-rays (within one year). If needed, we will take additional x-rays in our office.
• For your convenience, you may securely submit your patient registration form online prior to your scheduled appointment.

IMPORTANT: ALL PATIENTS UNDER THE AGE OF 18 MUST BE ACCOMPANIED BY A PARENT OR GUARDIAN AT THE INITIAL CONSULTATION VISIT.
