Pre-surgical Telehealth Consultation

To schedule a telehealth consultation with our office, please note that these appointments are exclusively for wisdom teeth (third molar) removal patients and require a panoramic image or other supporting dental images.

Your pre-surgical telehealth consultation includes a comprehensive evaluation of your dental needs, as well as a thorough review of any medical conditions and current medications. We also encourage you to discuss any concerns you may have prior to your surgical procedure during this time.

The following steps will help you come prepared for your first appointment with us:

• Please bring all insurance (dental and medical) information with you.
• At your request, your referring dentist may send current x-rays (within one year). If needed, we will take additional x-rays in our office.
• For your convenience, you may securely submit your patient registration form online prior to your scheduled appointment.

IMPORTANT: ALL PATIENTS UNDER THE AGE OF 18 MUST BE ACCOMPANIED BY A PARENT OR GUARDIAN AT THE INITIAL CONSULTATION VISIT.
